* Standard PCE (Pooled Cohort Equations) is the 2013 ACC/AHA 10-year ASCVD risk calculator used in most clinical settings today. It estimates the chance of a heart attack or stroke over the next 10 years using nine inputs: age, sex, race, total cholesterol, HDL, systolic blood pressure, BP medication status, diabetes, and current smoking. The formula is a race- and sex-specific Cox proportional-hazards model derived from the ARIC, CARDIA, Cardiovascular Health, and Framingham Offspring cohorts.
CardioRisk+ uses this same baseline and then adjusts it for risk markers that PCE doesn't capture — ApoB, Lp(a), TG:HDL ratio, A1C and fasting insulin (in non-diabetics), and the strength & timing of family history of premature CHD. The waterfall above shows how each adjustment moves the score relative to the PCE baseline.
